AK are the most common precancerous skin lesions among light-complected individuals [1], [2], [3], [4], [5], [6], [7], [8], [9], [10], [11], [12], [13], [14], [15], [16], [17], [18], [19], [20], [21], [22], [23], [24], [25], [26], [27], [28], [29], [30], [31], [32], [33], [34], [35]. Bowen's disease is defined as intraepidermal SCC with dysplasia at all levels of the epidermis. Controversy exists as to whether these lesions are precancerous, with the risk of future transformation into SCC, or whether they actually represent early malignant transformation. Bowen's disease is also known as SCC in situ.
History
John T. Bowen of Boston described two patients with SCC in situ of the buttock and of the lower leg, respectively [30], in 1912. He predicted that these lesions had “imminent”

2021, Journal of the American Academy of DermatologyCitation Excerpt :Histologically, AKs are characterized by epidermal hyperplasia with degrees of cellular atypia that approach those of squamous cell carcinoma (SCC) in situ but stop short of full-thickness epidermal atypia.6 Variable amounts of inflammation accompany AKs and this is reflected in the varying degrees of patient-reported sensitivity, from no perceived discomfort to extreme sensitivity.9-11 Pain can be associated with AKs but it is also a symptom of SCC; therefore, tender AKs should be assessed carefully.7,12

Besides sun exposure, risk factors for these in situ tumors are not well characterized (Butani et al., 2005). Several familial syndromes predispose to SCSC and other NMSC, which suggests that inherited genetic factors are involved in their etiology (Tsao, 2000; Somoano et al., 2005).
